Complete the form with the fullest amount of information you know about your military service. The Service officers can assist you, or you can send the completed SF 180 to:

If the military member was discharged, deceased or retired after 2004, their records are with their specific branch of Service. Send the completed form to the specific Branch Personnel Center.
