Complete the form with the fullest amount of information you know about your military service. The Service officers can assist you, or you can send the completed SF 180 to:

If the military member was discharged, deceased or retired after 2004, their records are with their specific branch of Service. Send the completed form to the specific Branch Personnel Center. Military Branch Specific Personnel Records
